How to Request Your Medical Records

Welcome to the SportsCare Physical Therapy Medical Records Request page. This page is designed to help both patients and attorneys obtain copies of medical records quickly, securely, and in compliance with HIPAA privacy regulations.

Patients can submit a secure online request form to receive copies of their own records or records on behalf of a family member. Attorneys can request records on behalf of a client by submitting a signed HIPAA authorization to our dedicated medical records fax line. Detailed instructions for each are provided below.

To protect your privacy, proper identification and a signed authorization are required before any records can be released. Requests sent directly to another healthcare provider for continuing care are processed at no charge. All other requests may be subject to a reasonable copying fee in accordance with New Jersey Department of Health guidelines. Most requests are processed within 7 to 10 business days.

Attorney Requests

If you are an attorney requesting medical records on behalf of a client, please fax a signed HIPAA-compliant authorization along with your request to the fax number to the left. Your request should include the patient’s full name, date of birth, dates of service, and the specific records being requested.

Attorney requests are typically processed within 10 to 15 business days. A copying fee may apply in accordance with New Jersey Department of Health guidelines.

Patient Requests

If you are a patient requesting your own medical records, or a parent or legal representative requesting records on behalf of a family member, please complete the secure online request form to the right. You’ll be asked to provide identifying information, the dates of service you need, and a brief description of the records you’re requesting.
