SportsCare Physical Therapy

Medical records

Your records are yours. Here is how to get them.

Federal privacy law gives you the right to see and copy your physical therapy records. Patients request them online below; attorneys and other third parties use the fax line. Most requests are ready within 7 to 10 business days.

Before you start

Four things the form will ask for.

The form takes a few minutes. Having these to hand means the clinic can act on the request the day it arrives instead of writing back for details.

  1. 1

    Who you are

Your full name, date of birth, and a photo ID if you are asked for one. Requests for a family member need proof that you may act for them.

  2. 2

    Where you were treated

The clinic or clinics you visited. Your records live with the clinic that created them.

  3. 3

    Which dates

The dates of service you need, or a range. “Everything” is fine, but a range is faster.

  4. 4

    Where they should go

To you, or directly to another provider, an insurer or an attorney. Records sent to another provider for your continuing care are free.

Patient request

Request your records online.

For patients requesting their own records, and for parents or legal representatives requesting on behalf of a family member. The form is hosted on a HIPAA-compliant service and your submission is encrypted.

FAQ

Common questions

How long does a records request take?
Most patient requests are processed within 7 to 10 business days. Attorney requests are typically processed within 10 to 15 business days. If you need records for an appointment with another provider, tell us the date and we will do what we can to meet it.
Is there a fee?
Records sent directly to another health care provider for your continuing care are free. Other requests may be subject to a reasonable copying fee in line with New Jersey Department of Health guidelines. If a fee applies, the clinic will tell you the amount.
Can I request records for my child or a family member?
Yes, as a parent or legal representative. Use the same form and indicate whose records you are requesting and your relationship to them. We may ask for documentation of your authority to act for an adult family member.
I am an attorney. Can I use the online form?
Attorney and other third-party requests go by fax to 973-887-9118, with a signed HIPAA-compliant authorization from the patient. Include the patient’s full name, date of birth, the dates of service, and the specific records being requested.
Can I just see my records without getting a copy?
Yes. Federal privacy law gives you the right to inspect your records as well as to receive a copy. Use the same form and say which you want. Our Notice of Privacy Practices describes this and your other rights in full.
What if something in my records is wrong?
You have the right to ask us to correct health information you believe is incorrect or incomplete. Ask the clinic that treated you for a Medical Record Amendment/Correction Form. We may decline in the limited circumstances set out in our Notice of Privacy Practices.
