Patient center
Everything before, during and after your visit.
What the first appointment is like, what to bring, how insurance works, and how to get your records — all in one place.

Your first visit
An hour that starts with listening.
- 1
A conversation first
Your therapist takes a full history — what happened, what hurts, what makes it better or worse, and what you want to get back to. Bring anything that helps tell the story.
- 2
A hands-on examination
Strength, range of motion, flexibility, posture, balance and coordination are measured, so there are numbers to compare against as you improve.
- 3
A plan you leave with
Treatment starts the same day: hands-on work where it helps, and a first set of exercises to do at home. Sessions typically last about an hour.
Please arrive about 15 minutes early for your first visit so check-in does not eat into your session.
What to bring
The short list.
- Photo ID — a driver's license, state ID, school ID or passport
- Your insurance card
- Any prescription or referral from your physician, if you have one (no referral is needed to start)
- Imaging reports — X-ray, MRI or CT — and any relevant medical records
- A list of medications you take
- Comfortable, loose clothing and sneakers, with easy access to the area being treated
Bringing a child under five, or a child with special needs? Fill in the pediatric medical history form before you arrive.
Insurance
Verified before you are treated, at no cost.
Most health plans cover physical therapy. Before your first visit, the clinic contacts your insurer, confirms your coverage and tells you what to expect — copay, deductible, any visit limits — so there are no surprises.
The fastest way to start is to book: verification happens as part of scheduling. Or call the clinic you plan to visit and the front desk will run it for you.

Forms and records
The paperwork, without the runaround.
Medical records
Federal privacy law (HIPAA) gives you the right to see and copy your records. Patients request them through a secure online form; attorneys use the records fax line. Most requests are ready within 7 to 10 business days.

If paying for care is a genuine difficulty, tell us. A financial hardship request is reviewed privately, and the outcome never affects the care you receive.

Common questions
- Does health insurance cover physical therapy?
- Most plans do, usually with a copay or after a deductible. Before your first visit the clinic verifies your benefits at no cost, so you know what to expect before you are treated.
- Do I need a referral?
- No. New Jersey, New York and Florida all allow direct access to a physical therapist. Some insurance plans still require a referral for coverage — the clinic checks this when it verifies your benefits.
- How many visits will I need?
- It depends on the diagnosis and on you. Your therapist sets a plan at the first visit and re-measures as you go. If you are not improving after a few visits, the plan changes.
- Will treatment hurt?
- Some treatment is uncomfortable, and some soreness after a session is normal. Your therapist will listen to your concerns about pain and keep discomfort to a minimum — tell them if something is too much.
- How long is each session?
- Typically about an hour, adjusted to your needs and your condition.
- Can I switch clinics partway through?
- Yes. SportsCare is one practice, so your records and your plan move with you across any of our 64 clinics.
